Transportation Assistance Application —

Dial-A-Ride or Valley Metro Ticket

The Brain Injury Alliance of Arizona has the opportunity to provide Dial-A-Ride or Metro Valley transportation passes to brain injury survivors at-risk or struggling with addiction, substance misuse or who are in recovery.

This program is in its trial phase and is currently only available in the Phoenix metro area, and only provides passes for certain transit systems.

We are not able to pay for or reimburse for Uber, Lyft, or taxi cabs.


Dial A Ride or Valley Metro passes provided by BIAAZ can be used for any of your daily activities. Trips are not limited to medical appointments or recovery meetings but can be used for any activity that helps you maintain your quality of life – from grocery store trips to visiting friends and family.

We are currently limiting passes to one 30-day pass per person, per month. We need to keep records of where every pass goes, so please provide your information below.

Where are you located? NOTE: This program is currently only available in the Maricopa County metro areas, and only provides passes for certain transit systems.

If there is another transportation assistance option you are looking for, please let us know. If there is a specific transit system or shuttle service you need, please tell us the name. We may be able to accommodate you in the future. WE ARE NOT ABLE TO PAY FOR OR REIMBURSE FOR UBER, LYFT, OR TAXI CABS.
